GENERAL REMARKS

The period under review in this journal has been mainly occupied with refit and docking of this ship, during which time short and long leave has been granted to all Officers & Men, which has benefitted them all very considerably.

A great number of alterations and additions have been carried out in the ship, improving certain living conditions.

The ventilation has been greatly improves, the changes forecast in the last quarters journal having come to pass : so that now the Mushroom heads just abaft the Breakwater on the F'castle have been covered in so that they now have a protection from water, but not from air, and by this means it is hoped that they will be able to remain open at sea and thus the air supply to the Sick Bay and its annexes and the Ship's company heads will be assured : this should be a great improvement. Further, Punkha Louvre ventilation has been installed on all mess-decks, which should make the living conditions more pleasant. And finally, 10½" supply and exhaust fans have been put in-place of the 7½" fans originally installed to provide a better supply of air to the Forward and After Medical Distribution Stations, and to ensure a more efficient exhaust of air which was considered necessary, especially if Anaesthetics should be given in either station at any time.

The lighting of both Medical Distributing Stations has also been improved : existing lights being made water-tight and more secure : whilst two special hea lamps, moveable on hooks - have been installed in each station : and these lamps will be supplied from a battery in a corner of each station, and so be independent of the main supply of the ship and available for use as secondary lightin, should the main supply fail in action or at some other time.

In the Dressing Stations above the actual Distributing Stations - are being places a block of points, from which lights on wandering leads are run, which can be used for treatment of burns by the Tannic Acid method : or as hot water bottles when covered.

It has been noted that there was no prophylactic Ablusion cabinet in this ship : and experience has shewn that men are averse to using the Ablution Cabinet attached to the Sick Bay for prophylactic irrigation. There is no room for one in the main "heads" of the ship, and so now one has been built in after end of the Port Night Heads on the Battery Deck, and should be useful, as it is easy of access to all men coming off from shore. The usual instructions on irrigating and contained in a frame, have been put up in the Cabinet, and all is being carefully looked after by the Captain of heads under supervision of the Sick Berth Staff.

The Sick Bay and annexes have all been painted out : and new Corticene laid on the decks.

During the period, when the ship was alongside - and not in dock - the full use of the heads was permitted, but whilst in dock Shore Heads had of course to be used : both Officers and Men's heads were improvised structures, the former being chemical ones and the latter flushed by water every 20 minutes from 0600 to 1700 : and then not so frequently. Both worked quite satisfactorily. Washing was carried out in the bathrooms on board as usual : and also in wash places ashore.

A small Marathon race and regatta was organised at the beginning of this period, and training for these events did many men a great deal of good. There have also been frequent games of both football and later cricket.

More volunteers have been called for to act as Blood Donors and for Anto-tetanic injections : the former have all been grouped and Wassermans tested, whilst the latter will shortly be receiving their injections.

A very large number of both Officers and Men received Anti-tetanic injections during last quarter : and then 14 days after the second injection, a specimen of blood was taken from 30 men, who had been inoculated, and all specimins were forwarded to Dr. Mc'Lean, St. Mary's Hospital, London. He very kindly tested the immunity developed, and found that this was satisfactory, then

not as high as a collection of Medical Students inoculated with the same batch of vaccine about the same time. Dr. Mc'Lean personally shewed me both lists, and I noticed that almost all the Medical Students had had Influenza between the two A.T. injections, whilst none of our cases had : and suggested that this attack might have had a "shock" affect - and so raised the immunity. It was agreed that this was a possibility. More specimins of blood taken from inoculated men will be sent later : and it will be interesting to observe if these work out in the same way. It is understood that the French give 3 A.T.T. injections ; but of course th French solder is much more exposed to the risk of Tetanus than the British sailor.

After the refit and docking period, the ship spent some time in hot climates at sea, and it was gratifying to note that the ventilation generally in the ship, and particularly in the Sick Bay, was much improved.

White uniform was worn during the last week of the quarter : which included tropical rig.

All Officers and Men were inoculated against the Typhoid group of diseased during the last part of June : reactions were remarkably absent.

The weather experienced during this period has been remarkable for its fineness, beautiful hot sunny days being the usual state of affairs. Some rain has of course occurred, especially in the earlier part of the quarter, but the latter time has been noteworthy for the continued fine weather.

During the hot spell, some of the offices have become hot, the temperature rising to 85° - 90° in the Ship's Office : an added element comes into play here, as this office is one of a group situated above an Engine Room - without lagging - and dependent on artificial ventilation.

It is understood that this has been noted and discussed before, when the ship was on the Mediterranean Station in recent year : and in this climate, it is only an occasional and temporary occurrence. As the weather became hotter still, this office was moved up to the Staff Office on the deck above, which provided fresh air and much improved conditions.

The ship has been kept in a very hygeinic condition. No rates at all have been seen during this period ; but a few cockroaches have come to light from time to time, and stringent efforts have been made to exterminate these pests - and to keep them down everywhere.

Health talks have been given to individual men - and to groups of men from time to time, including 1 lecture given to all Cook ratings on 10/6/40., on the subject of bodily cleanliness and the necessity for their having scrupulously clean hands, when preparing food of any sort, and another lecture broadcast to all Officers and Men on 27/6/40., on sunbathing and kindred subjects, and the necessity for care in sub-tropical weather.

The question of the ratings in the ship instructed in First Aid has been fully gone into. Some of these ratings have been drafted away from the ship : and arrangements have been made for their relief to receive full instruction in First Aid : and they are all now undergoing a course of instruction : several lectures have already been given.

Medical parties have been instructed as requisite and exercised : and all First Aid ratings throughout the ship are to receive a revision of their theoretical and practical work at their action stations.

First Aid Notes have again been placed on all notice-boards, and all men have been urged in Daily Orders to read them frequently. A list and diagram of the positions of all stretchers and First Aid bags and tins has also been put up on all notice-boards throughout the ship : and the attention of all men drawn to this : a diagram is enclosed.

The general health of all officers and men has been excellent. A large number of cases have been sent to hospital - more than usual - because for so many weeks the Sick Bay was in the hands of the Dockyard, and so unfit for nursing any cases in : and to a somewhat less degree, this also applied, whilst the ship was in dock.

There has been no marked prevalence of cases of any particular type except that a few cases of Tonsillitis occurred, during the very fine weather when the ship was in dock : due no doubt to the heat and dust. All officers and men were at once put on to gargling Potassium Permanganate solution night and morning : and this ceased very soon with this and the ship leaving the dock for sea.

There have been a few cases of infectious disease : 2 Rubella remaining from last quarter and Sick on Shore:

[Name Omitted] Corporal R.M. Age 22. Off. No. [Ommitted], both to duty 5/4/40. - : and 4 Rubella later in April, and one more case at the end of May. The first six cases were undoubtedly products of the epidemic which was so prevalent in the Fleet and the whole country during the spring of this year : and the latter case one of the remains of it. Full disifectant and inspection measures were taken with all cases.

One case of Mumps occurred on 6/6/40. :-

The source of infection was unknown. Full disinfectant measures were taken as requisite : and contacts have been and are being inspected.

Five Midshipmen R.N.V.R., from H.M.S. King Alfred - joined the ship on 10/4/40., being reported as Contact C.S.M. As reported earlier under their case notes : cases 12, 13, 14, 15, 16, 17 they were sent to hospital for throat swabs : all were returned negative, and no further trouble ensued.

Nine fresh cases of Gonorrhoea - and none of Syphillis have occurred during the period under review : and details of them are given on an attached sheet. Men have again and again had it and after intercourse : and a very large number of condoms have been given out.

It must be mentioned that a landing party - known as H.M.S. Primrose - was sent from this ship to join the Norwegian Expeditionary Force. Of those that returned to the ship four men were granted Hurt Certificates for wounds received in action ; they are all described under the individual Hurt Certificates. It was considered that the granting of these Hurt Certificates was warranted, and as there was no authority in H.M.S. Primrose in a position to award them, they were given from this ship on the return of the party, when full facts were obtained.

Consideration of Diseases

Gonococcal Infections (acute).
11 cases shewn : only 9 of these were fresh cases during this quarter, the other two being old cases from last quarter, re-admitted for discharge to hospital. Thus in resume on Veneral Disease given in these remarks, only nine fresh cases are mentioned.

Gonococcal Infection (sequelae).
1 case - Balanitis developed whilst under treatment for Gonorrhoea and Remains.

Diseases Caused by Metazoan Parasite.
4 cases - all Scabies : 3 to duty - 1 to other ships, this case being sent to R.N. Barracks, Devonport, for treatment, as none was possible on board this ship at that time. owing to being in Dockyard hands.

Disease of Spinal Cord.
1 case - Disseminated Schlerosis - to hospital.

Other Nervous Diseases including Mental. 5 cases -
1 Herpes Brachialis - to duty : 4 cases, 1 Psychoneurosis, 1 Psychasthenia, 1 [illegible] Neuritis, 1 Trigeminal Neuralgia - all to hospital. The case of Psychoneurosis was - [Name Ommitted]. Age 21. E.R.A. [Service No. Ommitted], who was sent to Royal Infirmary, Perth, Sick on Shore on leave, and shewn as discharged to that establishment on the day on which he went sick.

Diseases of Eye.
1 Conjunctivitis from Sick on Shore to duty - and 1 Detached Right Retina to hospital. The latter case was case 21 in this journal [Name Ommitted]. Age 19. A.B. [Service No. Ommitted].
He is shewn under Disease of Eye, because there was no history of trauma, and he was definitely classified in hospital as Detachment of Right Retina Idiopathic.

Diseases of Ear.
1 case - Rt. Otitis Media - to duty.

Diseases of Arteries.
1 case - Essential Hypertension - to hospital.

Diseases of Veins.
1 case - Varicose Veins - to hospital.

Diseases of Blood.
1 case - Lymphadenitis - to duty.

Other Diseases of Respiratory System.
3 cases - 2 Pneumonitis, 1 Convalescent Aberrant Pneumonia, all to hospital. These cases are shewn under this heading, because they could not be definitely be said to be of Pneumococcal Infection, especially the last case, which was only discovered and diagnosed by Radiological examination.

Diseases of Mouth, Palate, Fauces and Pharynx.
2 cases - 1 Ulcerative Stomatitis - to hospital : 1 Quinsy Rt. - Sick on Shore - to duty.

Other Diseases of Stomach.
4 cases of Gastritis - 1 remaining from previous quarter - 3 cases added - 3 to hospital - 1 from sick on shore. - 1 to duty from sick on shore. 1 case of Gastro-duodenitis to hospital.

Other Diseases of Intestines.
3 cases - 2 Constipation, 1 to hospital, 1 to duty : 1 Acute Retention of Urine [intelligible] to hospital : case 59 of this journal - [continued on Sheet no. 46]

[Name Ommitted]. Age 24. A.B. [Service No. Ommitted]. This diagnosis was given with the case sent in to hospital - and is retained, because in spite of repeated applications, no confirmation or alteration was obtained from hospital.

Diseases of Rectum & Anus.
2 cases - Haemorrhoids - Q to duty and 1 hospital. These comprise the same man with 2 entries : case 76. [Name Ommitted]. Age 36. Chief Shipwright. [Service No. Ommitted].

Diseases of Liver.
1 case, to hospital. This wsa case 71 in this journal -
[Name Ommitted]. Age 41. [Service No. Ommitted]. As the differential diagnosis in this case were several, and the final one dependent on the result of investigations made in hospital, and no diagnosis could be obtained from hospital, in spite of application, this diagnosis cannot be amplified, it is regretted.

Other Diseases of Generative System.
2 cases - 1 to duty - 1 to hospital.

Disease of Muscles, Fascae, Tendon and Bursae.
1 case - Pleurodynia - from Sick on Shore to duty.

Other Diseases of Skin.
7 cases - 3 to duty, which were - 1 Cheiropompholyx - 1 Cellulitis Left Ankle - 1 Whitlow Right Hand. : 4 to hospital, which were 3 Whitlows and 1 Cellulitis Right Hand.

Urinary Disorders.
3 cases - both to hospital, and both diagnosed as Haematuria in ship and hospital + 1 Retention of Urine (case 59).

N.A.D.
6 cases - all to hospital. 5 cases were Midshipmen R.N.V.R. - contact C.S.M. - and 1 case which was case 8 in this journal. [Name Ommitted]. Age 35. Marine. [Service No. Ommitted].
